Within this page are the forms you will need to fill out to begin your Debt Resolution Program. Print the forms, fill them out as complete as possible, and then mail them to the address on top of the Escrow Agent form.

In order to start the Program Right Away you can fill out the forms and scan them into .pdf format and email them directly to us at CustomerService@payonlinetoday.com. These forms can be taken to most office supply stores for scanning if you do not have a scanner at home.

These forms are in PDF format and Adobe Acrobat Reader is needed to view and print them. If you do not have Adobe Acrobat Reader please click on the Adobe link and download this free, common and widely used program.

If you have questions about filling out the forms, you may call the APT office at (877) 272-9232. If you have counseling questions, please call the Settlement Provider Directly, which can be found on the downloaded form.
